Community Event Request Instructions

Request for Participation in a Community Event

Welcome to Metro Public Health Department’s (MPHD) site to request participation in a community event. MPHD supports and encourages community efforts to improve the health and well-being of all of Nashville’s citizens. The department will make every effort to assist you and/or your organization with your event; however, we are not able to staff all requests. Submitting a request does not guarantee attendance. Final decisions will be based on staff availability and event logistics.

To initiate the request process:

  1. Please provide the name and contact information of the sponsoring agency/organization and the name of the contact person.
  2. Please submit your request at least 30 days prior to the event.
  3. Please specify the services that you are requesting from the MPHD.
  4. Please provide as much information as possible about the target population who will be attending your event.

Name, Phone, Email, Event Date, Event Location, Number People Expected and Description of Target Population are required fields.
