Infectious Disease Reporting

The diseases and events listed in the Infectious Disease Reportable Diseases List are declared to be communicable and/or dangerous to the public and are to reported to the local health department by all hospitals, physicians, laboratories, and other persons knowing of or suspecting a case in accordance with the provision of the statutes and regulations governing the control of communicable diseases in Tennessee and HIPAA.

How to report a disease

To report a sexually transmitted disease, please call: (615) 340-5647
To report a Tuberculosis case/suspected case, please call: (615) 340-5650
To report a vaccine preventable disease, please call: (615) 340-2168

To report all other diseases, please call: 615) 340-5632  fax: (615) 340-8914

Please complete a PH-1600 form and send by fax to (615) 340-8914.
