WE WOULD LIKE TO SCHEDULE THIS PROGRAM (please check one; please use a separate form for each type of program you would like to schedule):

Enviromutt (K-2nd grade; 30 minutes; large groups may be scheduled)
Curby's Reading Program (K-2nd grade; 30 minutes; 30 students maximum/program; you may schedule up to 4 programs/day)
Curby's Waste in Place Roadshow (3rd and 4th grade; 30 minutes; 30 students maximum/program; you may schedule up to 4 programs/day)
Trees Are Terrific (K-2nd grade; 30 students maximum/program; 30 minutes; you may schedule up to 4 programs/day)
Trees Too! (4th grade; 30 students maximum/program; 30 minutes; you may schedule up to 4 programs/day)
Hazardous Waste & Community Waste: Where does it go? (5th - 8th grade, classroom program lasts one full class period you may schedule up to 4 programs/day, OR teachers may check out the interactive model and programs materials for you to use in the classroom)
How the landfill works: Solid waste options (8th - 12th grade; one full class period you may schedule up to 4 programs/day )
Field Trip to the Recycling Center (all grades; one hour; limit 25 students/program)
