This online form is provided for your convenience in submitting a project for ADA review. All fields must be completed; enter N/A if necessary. When all fields have been completed, click the Submit button at the bottom.

This form is also available in a printable format (click here), which may be filled in online and printed for hard-copy submission, or may be printed out blank and filled in by hand.

Only one version is required - online or printed - not both.

Project Information

This form is to be completed by the project manager or requesting agency. Each project submittal form must be accompained by a final set of bid plans.

Project Submittal Forms that are incomplete upon receipt cannot be accepted and will need to be resubmitted. Please ensure you have provided all information below.

Relay Service 711

The State of Tennessee provides a relay service at 711 so people who use TTYs can have phone conversations with people who do not have TTYs.

This service is completely confidential, available to anyone 24 hours a day/7 days a week, for calls anywhere in the United States or its territories, at no charge to either person in the conversation.

Dialing 711 works whether or not the person placing the call is using a TTY, since operators who answer use voice and TTY.
