Applying for Long-Term Disability Benefits
If you are enrolled in LTD coverage and become disabled and cannot return to work, notify your supervisor or departmental Human Resources Coordinator as soon as possible. You, your supervisor, and the carrier will work together to process your claim. If you are transitioning from STD to LTD, you do not need to submit a new claim form.
