Naming and Changing Your Beneficiary

The beneficiary(ies) you name for supplemental life may be different from those you name for basic life and Metro pension benefits. Unless you designate otherwise, the beneficiary(ies) of your Metro basic life insurance will apply to your supplemental life insurance.

To name different beneficiary(ies) for supplemental life, contact Metro Human Resources. You may change beneficiaries for supplemental life at any time by completing a new Beneficiary Designation Form and filing it with Metro Human Resources. (Note: You may change beneficiaries for basic life and pension benefits as well through Metro Human Resources.)

REMEMBER: The person(s) you named last as your beneficiary(ies) will be entitled to receive your life insurance and pension benefits. That’s why it’s so important to make sure that your beneficiary designations are current.

If you fail to designate a beneficiary, or if your beneficiary dies before you do, benefits will be paid in the following order: to your spouse, your natural or adopted children, your parents or to your estate.
