Employee Flood Relief Application

1). LIKE SO MANY OTHERS, I DIDN’T HAVE FLOOD INSURANCE. HOW DO I APPLY FOR DISASTER ASSISTANCE?

Call 1-800-621-FEMA (3362) or apply online at www.disasterassistance.gov. Help is available in most languages.

What You’ll Need to Provide:

  • address of your damaged home,
  • the names of people living in your household,
  • a description of the damages,
  • insurance information,
  • your social security number,
  • a telephone number and mailing address, and
  • bank information.

You’ll be given a FEMA application number; keep that number handy. More information is available on the nashville.gov homepage.

Only damages not covered by insurance qualify for Federal Emergency Management Agency (FEMA) assistance.

2). I LOST MY PRESCRIPTIONS DURING THE RECENT FLOOD. DO I HAVE TO GO TO MY DOCTOR’S OFFICE TO GET A NEW PRESCRIPTION NOW?

Both Blue Cross/Blue Shield insurance and CIGNA insurance companies are aware of the recent floods, and both companies have agreed to temporarily suspend the need to obtain prior approval for pharmacy benefits.

For those employees covered by Blue Cross/Blue Shield insurance, call 1-800-367-7790.

For those employees covered by CIGNA, call 1-800-244-6224.

3). BECAUSE OF THE RECENT FLOODS, WILL MY PAYCHECK BE DELAYED? I REALLY NEED IT!

No, all payrolls were processed without delay, and you should receive your May 7, 2010 paycheck as expected.
