If You Are Injured on the Job

  1. Report your injury to your supervisor immediately. If your injury is urgent or life-threatening, please seek treatment immediately from the nearest hospital.
  2. Your supervisor will complete the necessary injury forms and submit them to ASC. You may need to sign these forms. Please coordinate with your supervisor accordingly.
  3. Before you receive services for your on-the-job injury, decide whether to use your IOD Network or Metro Group Insurance for your IOD injuries. For an IOD Network provider, an employee needs Employer's Authorization for Care (Red Card), Pharmacy Authorization Form, and IOD Report Form. For a Group provider, an employee needs IOD Report Form and Group Insurance Out-of-Pocket Reimbursement Form.
  4. Report back to your supervisor and/or case adjuster following your visit to the physician.

Treatment Options

Emergency Care
All Metro Nashville hospitals and many surrounding county hospitals are covered at 100% for IOD cases (see IOD Network Directory for a full list of providers).

IOD Network
The employee may seek treatment from an IOD Network provider. The employee incurs no out-of-pocket expenses under this network. All treatment, including pharmacy, is covered at 100%.

Group Health Insurance
The employee may seek treatment from his/her group health provider. The employee is initially responsible for co-pays and deductibles using this option. ASC will reimburse the employee for verified out-of-pocket expenses.
