Return Completed Poll Official Application and Document Copies
- Application Form
- Copy of Social Security Card
- Copy of Voter Registration Card or Driver's License
- Form I-9 (employment eligibility verification)
- Form W-4 (income tax withholding)
- Direct Deposit information for Payroll (ensures faster payment to you!)
- Appointment Subject to Approval by Election Commissioners of Political Party
- You will represent base on Primary Election Voting History
Print and fill out the following application, W-4 form and I-9 form to be considered for the Election Official position

- High School Poll Official Application (MS Word)
- W-4 form
- I-9 form
- Direct Deposit form
- You must include a clear/legible copy of your Social Security Card
- You must include a clear/legible copy of your Davidson County Voter ID card or valid Tennessee Drivers License
