Sexually Transmitted Diseases Testing and Treatment Program
The Sexually Transmitted Disease program provides confidential testing, treatment and counseling services for individuals who suspect they have or may have been exposed to gonorrhea, chlamydia, HIV and syphilis. The program is also responsible for contact investigation and partner notification in order to stop the spread of these diseases.
A legal photo ID and verification of insurance should be presented at the time of service. The cost of testing is $10.00. Individuals may call 615-340-5647 to obtain results via telephone or to schedule an appointment to discuss results in-person in the STD clinic.
The clinic moved to an appointment based system, October 1, 2018 to allow us to see individuals in a more timely manner. Walk-ins will continue to be seen but will likely take longer to receive services.
The registration form is available below for you to print and fill out prior to your visit, and then bring with you to your appointment. It will save you time during your clinic appointment.

Additional STD services are available at our Woodbine and East locations. Please note that clinic times are different and patients are required to sign-in by 2:30 p.m.

Patients are required to sign-in by 2:30 p.m. at all three public health centers. At times, services may need to be limited depending on staffing or demand for services.
This clinic observes the following holidays: New Year's Day, Dr. Martin Luther King, Jr. Day, President's Day, Memorial Day, Independence Day, Labor Day, Veterans Day, Thanksgiving Day and the Friday afterwards, Christmas Eve and Christmas Day.
