  • Process a last check (098) activity in EBS - see EBS Manual for Instructions
  • Have employee complete Exit Interview Questionnaire
  • Document and obtain all equipment and materials that belong to Metro
  • Submit termination paperwork if applicable to Central HR for personnel file
  • Submit Benefits at Separation of Service form to Central HR/Benefits
  • Contact Information Systems Help Desk (615) 862-4357 (HELP) to terminate email account
  • Obtain Cardkey from employee
  • Contact Randy Stafford (615) 862-5050 in General Services to terminate cardkey access
  • Complete the State of Tennessee Dept of Labor and Workforce Development Separation Notice and provide to Employee
