Community Development Block Grant (CDBG) - How's Nashville Assistance is a limited one-time payment towards a client's first month's rent, utility deposit, and/or security deposit. Once funds are exceeded for a client, case managers cannot submit additional requests for that client.

How to Submit a CDBG - How's Nashville Move-In and/or Utility Assistance Request
Case managers/Navigators must complete the CDBG - How's Nashville request form on hubNashville and upload all supporting documentation to the client's HMIS profile under "File Attachments."
Required documentation:
- CDBG Questionnaire Agreement form (required for all requests)
- Utility deposit bill/estimate or utility Denial Letter (required for utility requests)
- Welcome Letter, fully executed lease agreement, or tenant ledger (required for first month's rent and/or security deposits)
Incomplete requests or requests that do not have ALL supporting documentation uploaded to HMIS will not be processed and a new hub request will need to be submitted.
