Community health improvement involves organized efforts to enhance the health and well-being of communities through assessments, strategic planning, and collaborative initiatives. This process usually involves two main elements: a Community Health Assessment (CHA) and a Community Health Improvement Plan (CHIP).
The CHA is a process that identifies the community's key health needs and issues through data collection and analysis. It offers a detailed overview of the community's health conditions, including demographics, health results, and accessible resources. It serves as a foundation for developing targeted health and well-being interventions.
Based on the findings of the CHA, the CHIP is developed to outline strategies and actions addressing the identified health and well-being needs of our community. The plan gets revised every three years and requires teamwork among different stakeholders, such as public health agencies, healthcare providers, community organizations, and community members.
