The Community Action Team is a team of medical professionals, social worker, public health professionals, nonprofit partners, home visiting services, community members and others who develop and implement local initiatives to address recommendations made by the FIMR case review team and parental interviews. The Nashville/Davidson County Community Action Team and the MPHD Nashville Strong Babies Healthy Start Community Consortium have combined to form the Nashville Maternal Child Health Collective. Workgroups are created to address prioritized recommendations by developing action plans to implement prevention-based strategies.
