Mayor O'Connell has unveiled details of his transportation plan.
The Immunization Program provides routine child and adult immunizations. This program also works closely with pediatricians throughout Nashville to ensure that children age birth to 24 months are receiving the recommended series of immunizations.
Please bring any previous immunization records to all appointments. A parent or legal guardian should accompany any minor less than 18 years of age to all vaccination appointments. If you are the legal guardian, please bring all documentation including a court order and/or custody paperwork that supports guardianship.
Eligible children from birth through 18 years of age may receive free, federally-funded vaccine through the Vaccines for Children Program. However, an administration fee of $20 is charged for vaccines but will slide according to income.
Children from birth through 18 years of age who meet at least one of the following criteria are eligible to receive this vaccine:
If you have private insurance, private stock vaccine is available. Money is collected at time of visit by cash, check, or credit card. We can bill CoverKids but we do not bill private insurance at this time. However, we will provide a patient services ledger if you wish to seek reimbursement.
Free federal vaccine is available for uninsured adults age 19-64 for certain recommended vaccines. However, an administration fee of $20 is charged for vaccines but will slide according to income.
We can bill Medicaid and Medicare for adult vaccines. We do not bill any other private insurance at this time. However, if you have private insurance and you want to seek reimbursement then you may want to check with your company first and see if they will accept our Patient Service Ledger as proof of vaccination for reimbursement purposes. Money is collected at time of visit by cash, check, or credit card.
The clinics moved to an appointment-based system on October 1, 2018 to allow us to see individuals in a more timely manner. All patients should call and schedule an appointment for a visit. If you are more than 15 minutes late for your appointment, you will be asked to reschedule. Walk-ins patients are required to sign-in by 2:30 p.m. at all three public health centers. At times, services may need to be limited depending on staffing or demand for services.
The last time to sign up for walk-up services for Immunization Records is 4:15 p.m., but the phone is answered until 4:30 p.m.
