A Community Health Improvement Plan (CHIP) is an action-oriented plan outlining the priority community health issues based on the Mobilizing for Action through Planning and Partnerships community health assessment. The Healthy Nashville Community Health Improvement Plan outlines how these issues will be addressed, including strategies and measures to ultimately improve the health of Nashville by achieving our vision: A Healthy Nashville has a culture of compassion and well-being where all people belong, thrive, and prosper.
Thousands of Nashvillians participated in the visioning, assessment and strategy development processes that resulted in this Community Health Improvement Plan. The Mobilizing for Action through Planning and Partnerships process utilized the mayoral appointed Healthy Nashville Leadership Council as the Steering Committee, and Metro Public Health Department, Ascension Saint Thomas Health, Vanderbilt University Medical Center, Nashville General Hospital at Meharry, HCA Healthcare Foundation, Metro Social Services joined together to form a Core Team which was responsible for convening and guiding the process. The 2025 Nashville Community Health Assessment Report details the assessment process.
Community Priorities
The 2026-2028 Nashville Community Health Improvement Plan presented four key systems issues for the community to address during the next plan’s date period. Those priorities are Housing, Awareness and Navigation of Community Resources, Food Access/Food Insecurity, and Economic Opportunity and Job Skill Development.

Housing
Accessibility, availability, affordability and adequacy of housing can impact health in many ways. The 2026-2028 Community Health Improvement Plan sets a goal of using the Unified Housing Strategy as a roadmap to identify immediate low-barrier actions to improve equitable health outcomes for vulnerable populations.
Awareness and Navigation of Community Resources
Increased awareness of community resources means more people can navigate them and improve their health. The 2026-2028 Community Health Improvement Plan sets a goal of increasing the use of local resources to improve population health and well-being for underserved and vulnerable populations.
Food Access / Food Insecurity
The food we are able to put into our body plays a big role in our health. The 2026-2028 Community Health Improvement Plan aims to improve nutrition in Nashville/Davidson County by coordinating opportunities to address accessibility, affordability, availability and adequacy of healthy food in the community.
Economic Opportunity and Job Skill Development
Establishing a stable, sufficient income is key in maintaining good health. The 2026-2028 Community Health Improvement Plan set the goal of ensuring every Nashvillian can achieve long-term economic stability for a higher quality of life.
