A West Salem, Ohio woman has died as the result of today’s 1:20 p.m. collision on I-65 south near Madison involving a Dodge Journey minivan and a Jeep Wrangler.
The deceased is identified as Phyllas Louise Fone, 76, who was a back seat passenger in the Dodge.
The 56-year-old driver of the Dodge said a white or silver vehicle suddenly attempted to switch into his lane of travel and struck the right front tire of his minivan. The Dodge became disabled. The white or silver vehicle kept going. The 30-year-old Jeep driver said he saw the Dodge stopped on the interstate and immediately applied his brakes, but was unable to stop before striking the minivan from behind.
Fone died shortly after arriving at Skyline Medical Center. The driver and front seat passenger of the minivan, along with the driver of the Jeep, were transported to Skyline with non-life threatening injuries.
There was no evidence of impairment on the part of either driver.
